Hi Joshua, There is no book, the notes were somebody's personal research, the page numbers I think are just that of a document that contained research on multiple people.

Thank you, Tom. Is the full document available anywhere? Who is the compiler of these notes since the compiler in footnote 80 asks "Can anyone assist here?" ( not that I can be of any help) I assume he would not mind his name to be revealed.
